In 1999, the Department of Justice Civil Rights Division opened an investigation of the McCracken County Regional Jail (the "Jail") in Paducah Kentucky. The Department issued its findings in 2000, and on January 19, 2001, the Department filed a complaint and settlement agreement concerning conditions of confinement at the jail. The investigation was undertaken pursuant to Department's authority under the Civil Rights of Institutionalized Persons Act. At the time of the settlement, the Jail was a 350-bed facility.

The settlement agreement required the defendants to make numerous improvements to inmate health and psychiatric care, covering: intake procedures, medication management, infection control and crisis intervention, inmate housing, safety, protection from harm, and environmental practices.

Following the 2001 agreement, the Department continued to monitor compliance with the settlement until October 20, 2011, when the case was closed because the Jail had achieved compliance with the settlement's provisions.
